Diabetic neuropathy is one of the most common and most serious complications of diabetes mellitus. The cost of long-term treatment of diabetic neuropathy is the third biggest of all diabetic complications. The rational of diabetic neuropathy treatment should therefore arise from medical facts, and practical and economical view of the treatment should be taken into account, too. The basis of diabetic neuropathy treatment is hyperglycaemia control aiming to reach a long-term steady glycaemia level close to euglycaemia in combination with treatment procedures intervening with pathogenesis of neuropathy. The pain sensation in diabetic neuropathy is pathological (neuropatic pain) and needs early and specific treatment able to influence its pathogenesis.
