We present a case of 54 year - old woman who has been suffering for many years from undiagnosed celiacal sprue. The mentioned unrecognised condition resulted in hyperparathyroidism with severe bone symptomatology, anemic syndrome, generalized somatic astenia and hypovitaminosis. During one year lasting gluten free diet and supportive therapy general improvement of clinical symptomatology, as well as laboratory indicators, was observed, but normalization of some of laboratory signs was slow. Alkaline phosphatase levels normalized 12 month after complex treatment and parathormone serum concentrations remained slightly elevated even after one year lasting therapy. Control bone density remained at control examination after one year highly pathological. In discussion we briefly mention 4 other cases of celiacal sprue with signifficantly different clinical symptomatology. In two adolescent girls, pituitary nanism had to be excluded, in 1 case a immunity deficit with mouth ulcerations was present. One patient presented with mild clinical symptomatology (somatic asthenia ) and have had repeated abortions.
