Present neurohumoral approach of pathogenesis of chronic heart failure (CHF) has brought a new view about its origin and new possibilities of diagnostics, evaluation of the stage, prognosis and treatment of CHF by means of determining and inhibiting of activated specific and nonspecific hormonal factors. Therapeutic use of their blockers improved therapeutic results and bad prognosis of CHF. The best results were achieved by combination of betablockers and ACE inhibitors , angiotensin II receptors inhibitors with a small dose (25mg/day) of spironolactone and potassium saving diuretics. These procedures are involved in present national and international recommendations of modern treatment of CHF. However, there are gaps in wide clinical practice and we cannot be satisfied with the level of treatment of chronic heart failure.
