Unstable angina pectoris (UAP) and myocardial infarction without segments ST elevation (NSTEMI) is categorized to the group of acute coronary syndromes (ACS) with the same management. They are characterized by similar pathophysiological mechanisms, clinical symptomatology and ECG manifestations. Positive biochemical markers of myocardial damage are needed for the diagnosis of NSTEMI. Patients with UAP/NSTEMI represent significantly heterogeneous group and the right management requires thorough initial risk stratification. The article reviews basic pharmacotherapy with the focus on significant character of antiplatelet therapy and advantages of the early invasive diagnosis. Guides of optimal management are summarized in diagnostic and therapeutic algorithm.
