A course of massive pulmonary embolism in a 77-year-old patient with legs varices is reported. Shock and circulatory collapse developed soon after CT scan indicated to confirm the embolism. Cardiopulmonary resuscitation including external cardiac massage had started immediately. During resuscitation a thrombolytic agent was given. Regardless of seemingly ineffective resuscitation normal circulation was restored successfully in 30 minutes. Patient was discharged after a few days of convalescence, no worsening of quality of life was noticed. This case supports for initiation of resuscitation also in situations when a chance to save the life seems to be low. We would like to introduce a thrombolysis during resuscitation too.
