Thyrotoxic periodic paralysis is a rare endocrine disorder seen predominantly in Asian men. The occurrence in Caucasian patients is extremely rare. We present a case report of a man with reversible flaccid paralysis of his lower limbs and severe hypokalemia (1.38 mmol/l). After being examined by a neurologist, hypothesis of oligosymptomatic manifestation of thyrotoxicosis was formulated, which we proved by laboratory tests. Sonography of the thyroid gland revealed diffuse goitre. Treatment of thyrotoxic periodic paralysis includes correction of hypokalemia, beta blockers and antithyroid medication. Presented case report demonstrates the essential importance of cooperation between the internist and neurologist.
