Hypertensive patients with additional high cardiovascular risk factors constantly represent a particular therapeutical problem. Combined antihypertensive therapy will be indicated for them. According to available recommendations any treatment enabling blood pressure values to lower can result in reducing the absolute risk for these patients. Long-term treatment requires a different selection of antihypertensive medication particularly in patients with metabolic syndrome. Medication preference will include drugs without adverse effects on metabolism of saccharides, lipids or uric acid with respect to long-term antihypertensive treatment. From this point of view it is essential not only to reach optimal blood pressure control, but also to select a proper combination of antihypertensive therapy in above mentioned high risk patients with hypertension and comorbidities.
