The authors present the case of a patient who underwent aorto-coronary bypass surgery that was complicated by deep surgical site infection with osteomyelitis of sternum. They pointed out the cumulation of risk factors during hospitalization stay that led to a severe infectious complication necessitating plastic surgery reconstruction with musculocutaneous flap and vacuum assisted wound closure procedure. The authors emphasise the crutial urgency to have the knowledge of predisposing factors implicated in the etiology of infectious complications of cardiovascular surgical procedures by all attending physicians (surgeon, cardiologist, internal medicine physician). This knowledge leads to earlier detection of infection and prompt modification of prophylactic and therapeutical strategies
