The risk of perioperative complications is age-related and increases with concomitant comorbidities; it is higher in context of an urgent surgical intervention compared to an elective one. Severe perioperative complications in relation to elective surgery are rare in young, otherwise healthy individuals. This case report shows that major surgery might represent a considerable risk even in a healthy, relatively young person. Our patient underwent hip joint replacement and subsequently experienced a series of uncommon severe complications that eventuated into adrenocortical insufficiency. The recognition of respective complications and of the primary hypocorticism itself was not an easy matter due to their interacting and overlapping clinical presentations.
