Psychotic conditions, particularly schizophrenia, are commonly associated with severe chronic somatic diseases that might be the result of life standard of patients but also the result of antipsychotic therapy. The most frequent internal complications of antipsychotic therapy is hyperprolactinemia and following sexual dysfunction, metabolic syndrome, obesity, type 1 diabetes mellitus and hyperlipoproteinemia. Less frequent is prolongation of QT interval with risk of fatal dysrythmias and myocarditis induced by clozapin. Management of these complications involves careful selection of antipsychotics taking into account possible manifestations of hyperprolactinemia, monitoring of patient’s weight, glucose metabolism and levels of lipoproteins before and during antipsychotic therapy.
