The age of seniors - 65 years and more is not a contraidication of the beginning of renal replacement therapy. Early diagnosis of the kidney disease and subsequent dispensary of seniors in nephrological out-patient department is needed. Decision on way of renal replacement therapy depends on actual patient condition, on the presence of other diseases and complications, and also on patient cooperation, resp. his family, with the nephrologist. Conservative treatment, continuous ambulatory peritoneal dialysis, haemodialysis and renal transplantation are the various therapeutical forms of chronic renal diseases stage 4 - 5 in seniors. Senior`s age is not contraindication for renal transplantation. Mental quality of life of seniors is very good during the renal replacement therapy, but physical quality of life is decreased. Withdrawing of the dialysis therapy in elderly is possible only in the presence of generalisated atherosclerosis with dementia, in terminal stage of malignant irreversible disease and in the presence of many severe diseases.
