The new classification of chronic pancreatitis has being improved by new findings. In 2001 the TIGAR-O classification was published. It divides chronic pancreatitis according to its aetiological criteria. One of the forms of chronic pancreatitis is autoimmune pancreatitis (AIP) and it belongs to IgG4 diseases. It is characterised by the presence of increased concentrations of gamma globin IgG4 in blood serum and in tissues, the presence of auto-antibodies, and the presence of pancreatic fibrosis with lymphoplasmocytic infiltration and the absence of pancreatic calcification. The disease is commonly associated with other autoimmune diseases. It responses well to the treatment with corticosteroids Besides the characteristics of AIP and its diagnostic classification we also present the case study of a young patient with AIP and autoimmune cholangitis. It is clear that today we are increasingly confronted with a group of multisystemic autoimmune diseases, where autoimmune pancreatitis and its extrapacnreatic symptoms are included.
