The temporary discontinuation of the long-term oral anticoagulation therapy is usually necessary in case of trauma, a major surgery or some other invasive procedures. From the point of view of the perioperative management there is important to consider two principle risks: a) thromboembolic risk due to the interruption of anticoagulation and b) bleeding risk associated with a surgery or invasive procedure. This review deals with management protocols of the perioperative anticoagulation, so called bridging therapy, which were created on the base of the assessment of the patient´s thromboembolic and bleeding risk related to the surgery or invasive procedure.
