Mesenteric venous thrombosis is a rare disease with various clinical presentations. The abdominal pain is a typical symptom in the case of acute thrombosis, symptoms of portal hypertension is characteristic for the chronic one. In some cases chronic thrombosis can be unintendedly found during abdominal computed tomography or ultrasound. The CT scan is the main diagnostic modality. The case report is about a 56-year-old patient who was admitted to the hospital for subacute ileus. During the urgent operation liver cirrhosis and thickened distal ileum were found. PET CT was made after the operation and it demonstrated thrombosis of the superior mesenteric vein. This case was concluded as cryptogenic liver cirrhosis with portal hypertension and venous thrombosis with subsequent ileus. Despite the successful diagnosis and treatment sepsis and fatal multi-organ failure developed and the patient died.
