Toxic liver injury induced with herbal medicine (HHT) involves toxicity mediated with the effect of herbal medicine or
dietetic supplements (HDS). In the recent years, its incidence is growing due to increasing share of over-the-counter
products of herbal medicine and dietetic supplements to reduce the weight or to form the figure. However, a significant
part of HHT stays undiagnosed and unrecognised. In comparison with medicaments, HHT is most frequently the cause
of acute liver failure. The article provides actual overview of epidemiological data, main risk factors, etiopathogenesis,
present diagnostic and differential and diagnostic approach. The female sex, overweight, obesity and excessive
consumption of alcohol are the most severe risk HHT factors. Updated RUCAM classification is a gold standard in HHT
diagnosis. The creation of multinational L/HHT net (medical and herbal hepatotoxicity) could help to improve the prognostic
outlook of patients and increase their detection, as they are significantly underdiagnosed.
