Authors review modern approaches to therapy of nephrotic syndrome. They deny high proteins intake, emphasize the role of infection prevention and dietary habits adjustment. They are speciffic in describing therapy with corticoids, its modification and side effects. From cytotoxic immunosuppressive drugs the cyclophosphamid is regarded as the most effective drug. Cyclosporin A can induce similar type of dependance as corticoids. New immunosuppressives are underlined, most of all mycophenolate of mophetil, their side effects and need of biopsy before treatment. Antihypertensive therapy with the target BP 125/75 mmHg and lower with priority administration of ACE inhibitors (enalapril) is the basic of complex treatment. Wide use of plasmapheresis and apheresis of lipids is perspective.
