Acute tubulointerstitial nephritis(TIN) are in 15 % of all cases the cause of acute renal insufficiency, can change into chronic disease and thus they can be the cause of 25 % of terminal insufficiency of kidneys. Aetiology of TIN is a subject to immunological factors, congenital and hereditary causes, primary papilla impairment, bacterial infection of kidneys and other rather rare causes. In pathogenesis of the disease the IgE-mediated immunity (to drugs) is used and auto antibodies against tubular basal membrane. Diagnostic approach depends on aetiology; however, complicated imaging methods are commonly not capable to determine the right diagnosis. The authors point out the problem of TIN after antibiotics and Chinese herbs found mostly in girls longing for slim figure. TIN treatment is more empiric evidence – based treatment depending more on aetiology than rigid schemes.
