The authors present a case of a 1.5-year-old boy with pneumonia in whom neuroblastoma localized in back mediastinum was diagnosed during his hospitalization. The boy was admitted for febrile condition, in lab parameters higher inflammatory activity and leukocytosis dominated, chest X-ray disclosed presence of non-homogenous obscurity in the right mid pulmonary area – possibility of atelektasis or abscess. Regarding disputable X-ray finding CT examination of the chest was performed. It confirmed the presence of post inflammatory changes in pulmonary parenchyma and as the side finding paravertebrally present tumorous formation was detected. MRI confirmed the diagnosis of neurogenous hypervascularized tumor with tiny hypovascularized regions – cysts localized paraspinally left at the level of Th2-3 with the processus into the neuroforamen. Tumor was partially removed. Histological examination confirmed the presence of bad differentiated neuroblastoma.
