Introduction: Gastrointestinal perforations „in utero“ are a very rare surgical disease. Their incidence and estimated incidence is 1 : 30,000 births.
Case report: The presented case report refers a newborn born per section in the 35th gestational week, for imminent intrauterine asphyxia with Apgar score of 4/8, birth weight 3400 g, length 53 cm. Clinical and ultrasound findings were poor (fetal polyhydramnion, marked isolated ascites, abdominal distension), but suggesting pathology in the abdominal cavity. Surgical revision was performed after fetal paracentesis of the abdominal cavity which 100 ml of clear fluid with meconium residues were aspirated. It found a generalized form of meconium peritonitis with intestinal perforation in the ileum. Subsequent terminal ileostomy at the perforation site. After stabilizing the general condition of the patient, we performed surgery for meconium periorchitis, a rare condition of benign lesion as a result of intrauterine perforation of the intestine. At present, the patient is free from any inconvenience after closing the stoma.
Conclusion: It is estimated that at the time of his professional career a child’s surgeon or urologist will encounter the described diagnosis only once or not at all.
