Uterine artery embolization is today a standard method for the treatment of symptomatic uterine myomas. The procedure is safe, associated with only minimal number of complications. The most frequent one is transvaginal expulsion of the myoma. We present a case of a 32-year-old patient with symptomatic intramural myoma who preferred its conservative treatment. Therefore, uterine artery embolization was performed. The procedure was without any complications. Three weeks after the procedure the patient experienced a sudden onset of pain in both hypogastric regions and minor uterine bleeding. The diagnosis of transvaginal expulsion of necrotic myoma was made. Its transvaginal resection was performed. Six months later the patient feels well and is symptom free. Expulsion of intramural myoma and its cure can be thus considered as definite treatment and not a complication of the embolization therapy.
