Pregnancy reaching 42 weeks (294 days) is defined as post-term. Accurate pregnancy dating is crucial to the diagnosis. First trimester ultrasonography significantly reduced the number of post-term pregnancies in which the dating was based on the last menstrual period. There is no conclusive evidence that routine labour induction before 42 weeks improves maternal, foetal and neonatal outcomes compared to expectant management. After exclusion of risks factors, it would seem appropriate to inform pregnant women about possibility of expectant management or labour induction. Careful intra-partum foetal surveillance should be offered, irrespective of whether labour was induced or not.
