End-of-life discussion with a patient afflicted by amyotrophic lateral sclerosis is considered to be very delicate. In our part of the world the doctor usually tries to avoid this discussion reducing the communication with the patient to a short statement about the disease prognosis, then determines the therapy and leaves any the further destiny and course of the disease to the “nature”. The expansion of information sources, especially the internet, has led to a significant progress of the awareness of the patients and their families; the doctor-patient relation has been joined by an attorney. That is the reason why it is necessary to be prepared to start a discussion about this issue. The work introduces some of the medical, as well as the ethical and legal aspects of this problematic and also mentions several methods of how to solve them considering the actual situation in Czech Republic.
