Monogenic causes of ischaemic stroke represent a heterogeneous cluster of diseases whose common denominator is the impairment of critical site for the development of stroke. The diagnosis of these disorders has implications for their mapping in population, prognostic and therapeutic considerations as well as for genetic counselling for relatives. Due to their heterogeneity there is no uniform classification at present. They are usually classified according to underlying mechanism, mode of inheritance, and target structure of pathological process. Diseases affecting the structures of the vessel wall are usually classified according to the size of affected vessel. Thus we recognize small vessel disease, large artery disease and disorders affecting both small and large arteries. In our article we focus our attention on monogenic disorders affecting small vessels: CADASIL, CARASIL and HERNS.
