Objective: Kidney paired donation offers incompatible donor-recipient pairs the opportunity to match for compatible transplants. The outcome is an improvement in quality of life and survival. Materials and methods: In TC Banská Bystrica there has been a kidney transplantation program from live donors since 1994. We can see the increase of number of this type of transplantations. In 2006 they constituted 35 % of all transplantations. Despite the progress of this type of transplantation there still are some pairs who are ABO incompatible or cross-match positive and they cannot exchange kidneys. The „pool“ of pairs has been formed in 2004 that resulted in two cross transplantations. Results: Kidney transplantations from live donors have great results as in short-time so in ling-time graft survival. In our group 3-year-survival accounts for 90%. All four kidneys from the kidney paired donation program had early onset of the function of the graft and they all survive almost one year after transplantation. Conclusion: The exchange of kidneys between two live donors, who are ABO incompatible or cross-match positive is an alternative successful strategy for living donor transplantation.
