Objective: Buccal mucosa is the most frequently used tissue in the urethroplasty for urethral strictures longer than 2-3 cm. We describe our first experience with buccal mucosa using dorsal onlay urethroplasty Methods: Five patients with a mean age of 42 years underwent urethroplasty with dorsal onlay buccal mucosa graft in the year 2009 in our institution. Mean stricture lengths was 3.2 cm. Three patients had stricture after instrumental intervention or catheterisation. In 2 patients the cause of stricture was unknown. All 5 patients were treated with dilatation or inner urethrotomy in the past. Procedure was performed simultaneously by two surgical teams (one taking the tissue harvesting and the second performing urethroplasty). Result: The mean operative time was 95 minutes. No intraoperative or early postoperative complications were observed. Patients had urinary catheter inserted for 2 weeks. Three patients after urethroplasty are in good condition (uroflowmetry, urethrography). One patient had an infectious complication after surgery (local abscess) and one has a restricture. Mean follow up period is 10 months. Conclusion: Dorsal onlay urethroplasty with buccal mucosa graft is treatment of option in longer strictures with minimum commorbidity and good therapeutic effect
