Hypertension and diabetes mellitus increase significantly the risk of atherosclerosis development with wide-range of end organ damages. The paper is aimed at the currently valid pharmacological procedures in the therapy of high blood pressure in diabetics, which were created on the base of practical clinical experiences and results of epidemiological and clinical therapeutical studies. The target blood pressure value in diabetics is < 130/80 mmHg, and if nephropathy with daily proteinuria over 1 gram is presented, the target blood pressure value is < 125/75 mmHg. Diabetics have to be treated by combined nonpharmacologial and pharmacological methods from the beginning. ACE-inhibitors and antagonists of angiotensin receptors are the first choice drugs used predominantly in hypertensive diabetics. They very often have to be combined with β-blockers, diuretics and calcium channels antagonists.
