The good cardiosurgery results of cardiac diseases depend on a precise preoperative diagnosis, adequate surgical procedures, perioperative management and long-time postoperative care. The author wants to show the role of a cardiologist in perioperative care for adult patients with valvular and congenital heart diseases. The cardiologist assures the adequate “timing” of surgery (or reoperation) in a group of chronic patients, helps to manage the “emergency” of procedure in acute disease. He realises transoesophageal echocardiography examination at the operation theatre if it is indicated. During the post - intensive care period the cardiologist manages postoperative clinical status of patients and pharmacological or supportive therapy. He evaluates impact of cardiac surgery. Thw cardiologist also manages an adequate long-time care of patients after cardiac operations in regional centres.
