The techniques of revascularization treatment of hemodynamically significant atherosclerotic stenosis of renal artery reached relevant progress in the last years mainly by the means of standard implantation of stents. Despite this favorable development, correct indication of endovascular therapy of renovascular hypertension and ischemic nephropathy remains unsolved in clinical practice as alternative of drug treatment. With hope expected results of prospective, randomized clinical studies STAR and ASTRAL did not bring any definite conclusions from this point of view. They proved statistically nonsignificant differences within the comparison of clinical effectiveness of pharmacological and revascularization therapy. The ongoing CORAL study opens space for completion of so far equivocal opinions of indication of endovascular renal interventions. Until the study’s accomplishment, recommendations of American College of Cardiology/American Heart Association 2006 serve as relevant guidelines to the indication of implantation of renal artery stents.
