Recently the issue of aortic valve sparing operations gets at the forefront of cardiosurgeons. The tendency to perform aortic valve sparing operations is related primarily to younger patients, who are not exposed to risks resulting from the use of anticoagulation treatment after these operations. The choice of optimal surgical procedure on the aortic valve due to aortic regurgitation is not easy. The decision has to be done if to substitute the valve by long-term used and elaborated procedures with a small operative risk and known long-term results with the risk related to the use of anticoagulation treatment, or to perform sparing operation, which despite the higher technical demandingness we might perform without increased post-operative morbidity and mortality.
