Surgery always plays an important role in treatment of primary tumors of liver despite development of other treatment modalities. There are new approaches, procedures, ways and means used in surgery to reach better short-term and long-term results. The aim of the work was to present a review of current knowledge and experience with surgical treatment of primary tumors of liver. The authors added their own experience from the years 2004-2008. They presented a clinical case of 49 patients who underwent an operation from which twelve were operated for benign and 37 patients for primary malignant tumors of liver. The authors reached remarkable immediate post-operative results. Liver resection either anatomic or non-anatomic remains the essential and the most successful therapeutic method of benign as well as malignant primary liver tumors. In tumors where resection could not be applied other surgical procedures can significantly improve surviving of patients.
