The primary clinical goals of colonoscopic imaging developments are improved detection of neoplasia and in vivo histological diagnosis of detected lesions. These new technologies are discussed in the first part of this article /after some comments to quality exams/ according to the clinical problem they address. Second part of this comment points at colorectal carcinoma screening programme in Slovakia. Colorectal cancer achieves the first place as a common cause of cancer deaths in men and in women in Slovakia. Screening for colorectal cancer is effective in reducing incidence and mortality by removal of premalignant polyps and through early detection and treatment of cancer. Pre-pilot phase of colorectal carcinoma screening was realised in 2009 and reached the response rate of 56.4 % in selected population and 51 % acceptance between colonoscopists. The true prevalence of adenomas in the average-risk screening population is greater than 50 %, as defined by high-level adenoma detectors. These levels of detection probably can be achieved by any colonoscopist using modern sophisticated technique with high-definition colonoscopes.
