Foregut duplication cyst (FDC) is an extremely rare congenital developmental anomaly. It may occur in any part of
the alimentary tract, but the most common sites are oesophagus and stomach. The authors describe a case report
of a 30-year old man, who was diagnosed to have an extraluminal and expansively growing tumorous lesion arising
from the wall of gastric cardia. Histology revealed a cyst of 42x30x20 mm in size, the wall of which was composed of
the smooth muscle lining by columnar ciliated respiratory epithelium. It was immunohistochemically positive for thyroid
transcription factor TTF-1. Some epithelial cells contained intracytoplasmic mucin-filled vacuoles. Pathological findings
favored a diagnosis of gastric FDC. Control CT scan done 16 months after operation did not show a local recurrence.
Despite rarity, FDC should be included in differential diagnostic approach of the extraluminal tumorous masses of
the gastrointestinal tract. In biopsy practice, an evidence of the respiratory epithelium lining the cyst is a very specific
and crucial finding for pathologist to establish a correct diagnosis.
